Job Details:
The social worker is responsible for assisting patients and their families with the psychosocial issues that arise as a result of their hospitalization and/or illness. He/She conducts psychosocial assessments and discharge screening for hospitalized patients and acts on identified needs. Intervenes in crisis situations and assists staff with difficult families and/or situations. Provides discharge planning services, offering safe options for patients. Coordinates plan for services with the patient, hospital staff and community agencies. Advocates for the patient and empowers them to improve their quality of life. Reports to Director of the Medical Social Services Department. Must have Social Work license. Prefer medical social work experience
